The Social Service Designee at WCHC is a professional who works in our long-term care facility, to address the psychosocial, emotional, and behavioral needs of residents. The role supports residents and their families by assessing needs, coordinating services, providing counseling, and ensuring that medically related social needs are met in accordance with federal and state regulations.
